Nurse Practitioner

Healthy Horizons Clinic is seeking a dedicated Family Nurse Practitioner to join our team. In this role, you will provide comprehensive primary care to patients across all age groups. You will be responsible for conducting physical exams, diagnosing health issues, prescribing treatments, and managing chronic conditions. Additionally, you’ll perform minor in-office procedures such as suturing, joint injections, and Pap smears.

As a Family Nurse Practitioner at our clinic, you’ll play a crucial role in ensuring high-quality care and maintaining continuity for our patients. We are looking for a compassionate and skilled provider who thrives in a collaborative environment.

Key Responsibilities:

• Conduct patient evaluations, physical exams, and diagnostic tests
• Prescribe medications and develop treatment plans
• Perform minor procedures (suturing, joint injections, etc.)
• Manage chronic conditions and provide health maintenance
• Collaborate with the medical team to ensure optimal patient care
• Complete patient documentation accurately and promptly

Qualifications:

• FNP-C Certification (required)
• BLS Certification (required)
• 1+ year of experience as a Family Nurse Practitioner (preferred)
