About the role:

• Full-time, permanent position
• Monday-Friday 8a-5p
• FNP / PA will work in an outpatient setting seeing an underserved patient population
• Family Nurse Practitioner/ Physician Assistant will be responsible for Primary Care services helping individuals manage their chronic conditions, preventative care, and patient education
• APP will join a busy practice that offers a collaborative, team environment

Background:

• Board Certified Family Nurse Practitioner / FNP or Physician Assistant / PA
• 1.5-2 years of experience as a Family Nurse Practitioner or Physician Assistant preferred
